Are you a care professional, health visitor or midwife working with a family that could use our support? Before you can access our service, you must have a referral.
LEEDS BABY BANK’S REFERRAL PROCESS
We accept referrals from care professionals and voluntary organisations who are directly supporting an individual or family. We do not accept self-referrals.
Information for care professionals: To make a referral you will need to register with us and become a professional referral partner. To do this please fill out the form using the link below.

You can register as a referral partner using the Leeds Baby Bank registration form.
you’ll receive two emails from us
One email will welcome you to team (including T&Cs and a clear guide to the system) and another will have your login to our referral portal.
create a contact & submit in the portal
Once you’ve logged in to the referral portal, please ‘create a contact’ to submit your first referral for a family to Leeds Baby Bank.

Please note we will always contact the service user regarding the referral unless instructed otherwise. Please ensure you read and agree to our guidelines before putting in a referral.
To ensure we comply with England’s regulations we will confirm with the family that the child is already on the requested formula and it will only be consumed by the named child or children.
